Skilled Long-Term Care

Our Town short-term rehabilitation
Incorporating the innovative Our Town model of care, our short-term, post-hospital rehabilitation program includes physical, occupational and speech therapies designed to quickly get patients back on their feet, back home, back to work, or simply back to the highest possible level of functionality and independence. Respite care
For families who need a break from their daily caregiving routines, Deerbrook also offers short-term respite programs. Combining round-the-clock nursing supervision, restaurant-style dining and a regular program of social and recreational activities, respite care provides a safe and nurturing environment for seniors, and wonderful flexibility and peace of mind of their at-home caregivers. Care can be arranged for as little time as one weekend or longer, if desired.
Alzheimer’s Care
“The Village,” our separate, self-contained dementia care unity, combines specialized programming, personalized care planning, and 24-hour personal support that individuals with Alzheimer’s and related disorders need to maintain quality of life, retain personal dignity and maximize independence. Our small, close-knit community helps give those in our care an added sense of purpose, belonging and well-being. Compassionate staff members receive extensive training focused on the special physical, emotional, social and dietary requirements of our residents.
Hospice
Compassionate hospice support is among the many comprehensive services offered at Deerbrook. Consisting of palliative care, counseling and spiritual support for both patients and their families, our dedicated hospice professionals and volunteers provide comfort wherever it is needed – at home, in the hospital, or in our long-term care facility.

Social and recreational programs
Short-term and long-term residents alike are able to enjoy a full complement of social and recreational activities designed to keep hands nimble, bodies moving, minds active and spirits up.
Round-the-clock admissions
We understand the frustration and inconvenience involved with having to “wait around” for needed care. This is why we accept after-hours, weekend, and emergency admissions, 24 hours a day seven days a week. We also offer home and out-of-town screenings.
Discharge planning
Prior to discharge, our rehabilitative team makes sure patients are ready for the challenges involved with returning home or to an alternate level of care. This may include a visit at the resident’s home to assess possible alterations and adjustments that will enhance safety, accessibility and convenience. Social workers may arrange for wheelchair assistance or other assistive devices. Nutritionists may provide dietary counseling, and nurses will instruct residents and family members in wound care and medication management, if applicable.
Payment options
For optimum convenience and flexibility, Deerbrook accepts private payment, Medicare, Medicaid, Managed Care contracts and private insurance plans. Please call for coverage options that apply to specific programs and services.
The respite program is a private pay option, not generally covered by insurance.
